The High Value Care (HVC) curriculum has been a key initiative by the American College of Physicians (ACP) to improve the practice patterns of physicians. Including this HVC curriculum into residency training is an effective way to help residents develop medically appropriate, efficient and patient centered practice patterns. The Choosing Wisely® campaign is one of the most well-known initiatives of the American Board of Internal Medicine (ABIM) to reduce waste in the health care system and avoid risks associated with unnecessary treatment.
We created a HVC curriculum at our institution based on ACP’s HVC curriculum version 3.0. After completing all five lectures in a 3-month period, there was a need to continue revision of the key concepts. Medical jeopardy is a common method used to assess medical knowledge. Residents often perceive game based lectures as a more enjoyable and stimulating experience.
We introduced Choosing Wisely® Jeopardy during noon conferences once a month to revise HVC. The questions included a stem based on the Choosing Wisely® recommendations.
Example: Per AASLD recommendation, before performing paracentesis in a cirrhotic patient, you transfuse FFP to bring INR below ___?
Answer: Don’t routinely transfuse fresh frozen plasma and platelets prior to abdominal paracentesis or endoscopic variceal band ligation
Although Jeopardy cannot replace didactic lectures to educate residents about HVC, this format of game based lectures is a beneficial tool to integrate the recommendations of the Choosing Wisely® campaign into residency training.
